WORK PERMIT REQUESTS 

 

Follow the steps listed:

1.     Complete the work permit application and return to the career center.  Original signatures will be required.
 

2.     All 3 sections must be complete (student, employer, and student/parent signature).
 

3.     Students must have a 2.0 GPA to be eligible.  We will check your GPA.
 

4.     Once the work permit is issued a student will need to pick up in person (student signature required).  Report to the Career Center.

Students will receive 2 copies. 

One will be for the employer (turn in to supervisor) and one for student records.

Make sure a good working phone # is included to contact you with any questions.
